Send e-mail to Amanda, tell her you wish to order a Mike & Key ARC badge (ref# 149597), give her the name and callsign you wish on badge, delivery address, and credit card payment info (including security code on back of card). If badge is not required by particular date, shipping is free.

Order Online or Downloadable Form

Select Order Online, fill out all required information on form. In Group field use Mike & Key ARC. Select the back syle. In Name field put your handle or name. In Title field put your FCC Call Sign. Include credit card payment info (including security code on back of card).

Select Downloadable Form if you prefer to mail or email in your completed form with a check by mail or call-in the credit card information. Use the same instructions as for Order Online.

Mike & Key Amateur Radio Club Unofficial Name Badge

Unofficial club badge is available with the following conditions:
1. Plastic name badge with Handle/Nickname, call sign and full name (as listed on FCC database) is available for all current club members.
2. It is NOT an official badge, but the recipient is encouraged to wear it at the club meeting and/or club function.
3. There will be a signup list at the monthly meeting and a badge will be provided at the following month.
4. The badge needs to be picked up at the meeting as it will not be mailed (unless a SASE is provided).
5. The badge is made Pro Bono, but you are always welcomed to give a donation at any amount to the club treasurer (treasurer@mikeandkey.org).
6. Remember that it is lifetime guaranteed. If you are not satisfied you can return it and you will get DOUBLE the payment back (not your donation, however).
7. The badge is made of PVC size approx. 3.35 x 2.13 (inches)/ 85 x 54 (mm), 30 Mil thickness about same as typical credit card (CR8030), with a punched slot of approx. 15 x 3 (mm).
8. Metal badge clips with clear strap are provided, but the badge can be worn with a lanyard (not provided) such as other ID cards.
9. It is printed in black using thermal resin transfer ribbon on both sides of the card so that it can be read even if it is flipped around.
